Can someone please help me?? I want to file a civil complaint by fax. The court rules say that you have to say "by fax" under the title of each document....where is that exactly??? For example, for a civil complaint, where would I say "by fax" ?? Thank you!

It doesn't really much matter as long as it is below the name of the court. The simplest place to put it is before or after the word "Complaint."

On the first page, on the right side of the pleading, under the name of the document. For example, to the right of the case caption (Plaintiff X v. Defendant Y) your complaint should say "Case No.: (leave blank)" and under that, "Complaint for ____________." Under "Complaint" is where you should put "By Fax."

You also need to submit the mandatory facsimile cover sheet, MC-005, and make sure you provide a payment method.
